Register as a Mentee



I acknowledge that -

  • I confirm that I am above 18 years old. If I am less than 18 years old, I have taken permission from my parents/guardians before registering as a mentee and it will be my responsibility to ensure that my parents/guardians will be available during mentoring discussions with the mentors.
  • I am voluntarily enrolling in this mentoring program. I will be professional in the mentoring relationship as a part of this program.
  • I will be fully responsible for the outcome/result of this mentoring relationship.
  • I understand that the opinions/guidance/suggestions given by the mentors during this mentoring relationship are of their own and do not represent the suggestions/opinions/stand of Mentor Yuva.
  • I authorize Mentor Yuva to share my contact information (email, phone number etc) with the matched mentor/s and/or mentee/s. I understand that mentees, mentors or other Mentor Yuva volunteers may contact me.
  • The information provided by me in this form is accurate to the best of my knowledge.
